3rd, "Black Hawk" on Faint Harrah's Pattern,  (A barcode was affixed to back and card was hand scanned. The slot club was not promoted much unlike their other properties. The player really had to go out of his way to sign up! The last line on reverse is "Know when to stop before you start".)
